In addition to the problem of patient data collection and filing of medical records is an important thing to note. By using a web-based application, UPTD Pukesmas Lebaksiu Tegal regency, utilizing information technology, needs to be made application that can overcome the problems faced as high as possible to improve public health services for the realization of vision and mission of puskesmas. This application will be created by authors using Apache 2.4.29 as web server, PHP 7.1.12, Framework Laravel 5.4 as programming language and MYSQL 4.7.4 as database.
