The information service application using Location Based Service is a website-based application that has the function of making it easier for the public to obtain information on health services carried out by the Health Service. Due to the fact that there are still people who still do not get maximum health services, this application helps provide information quickly and in detail. The purpose of this study is to produce a health service search application, and aims to decentralize health service data from the Health Office. The application of the waterfall method in the development of this system will make this application very easy to develop, repair and also very easy to maintain. With a map, the community will be helped to find the nearest health service, so that information on health services provided by the Health Office will be conveyed to the community. This website-based application will be easily accessed by the wider community, it is enough to use the internet and a browser.
